NHS 111 Mental Health Crisis Care Services - National Patient Feedback Survey
Why have I been asked to complete this survey?
NHS England has published this survey for individuals who have accessed mental health services through the NHS 111 'select mental health option' service. Its purpose is to gather feedback on your experience and the support you have received.

This survey opened on 15 July 2024 and closes on 15 July 2025
How does this survey work?
The survey is an open invitation for anyone who has accessed NHS 111 via option 2 (Mental Health Support) from 15 July 2024 to 15 July 2025.
When you ring NHS 111 and select option 2 (Mental Health Support), you will be put through to the Mental Health Advice and Support Team (MHAST) and this service is run by Mind - the mental health charity.
Please note: The survey is aimed at individuals who have accessed mental health support via NHS 111 Mental Health option.
When you click on the link the survey suggests this is only for use if accessing crisis support, however please also complete the survey for any mental health support you have received via the NHS 111 option 2 method.
The drop down menu on the survey does not include Mind, therefore you will need to select Humber Teaching. Please ensure that you provide feedback on the services you have received by Mind if you did not get signposted to Humber Teaching NHS Foundation Trust services. If you did see a clinician face to face for crisis services because of your telephone call to NHS 111 mental health option, you will need to feedback on your experiences using both services.
How will my feedback be used?
Responses collected through this survey will be analysed to identify areas where improvements can be made to enhance the quality of care provided. Sharing your views and experiences helps us to continuously improve our services and provide the best possible patient experience.
