Why have I been asked to complete this survey?

We are keen to learn about the experiences of those who have been cared for on our Mental Health Inpatient units.

There is also a Discharge Survey to accompany the full survey.

When does this survey take place?

Full survey: Individuals will usually be asked to complete this survey during their admission.

Discharge Survey: You will be invited to complete this after you are discharged from inpatient services. It can be accessed via a QR code which staff will make available to you.

How do I access this survey?

The surveys are available via MS Forms which is accessed online via a link or QR code made available by the team caring for you. Paper copies can be available upon request, please speak to a member of your clinical team.

Responses are anonymous but as part of the survey you will be asked which unit you are being cared for on. This will enable us to link your feedback to the team looking after you.

What will happen to my feedback following completion of this survey?

Positive feedback will be fed back to the teams. Any areas for improvement which arise from feedback will be managed through an action planning process to help us promote a positive experience for you.
