Hull & East Riding Memory Assessment Service

How to access this service

The Hull and East Riding Memory Assessment Service accepts referrals primarily from GPs.  

The Service requires the consent of the person to be able to offer an appointment.  If the person is unable to give consent, the person holding Lasting Power of Attorney will need to give consent on the behalf of the person or, where this is not in place, the GP will be required to hold a Best Interests meeting prior to the referral.

If you are under the age of 65 you will first be referred to Adult Mental Health Services.

What should I do if I am concerned about my memory?

If you have a concern about your memory, it is important that you make an appointment with your registered GP. 

If you are a carer, relative or friend of someone who you think has memory problems, encourage them to see their GP at the earliest opportunity as there are other treatable conditions that can cause memory loss.

These treatable conditions need to be eliminated/treated before a referral to the Hull & East Riding Memory Assessment Service. Blood tests need to be taken and your GP may also refer you for a CT scan of your head to support the referral.

The Team

The Hull & East Riding Memory Assessment Service is a Specialist Dementia Diagnosis Service for people over the age of 18.

The team includes: Nurses, Support, Time & Recovery Workers, Clinical Psychologists, Psychology Assistants, Occupational Therapist, Consultant Psychiatrists and Health Care Assistants supported by a team of Administrative Assistants. The team works in partnership with the Alzheimer’s Society, Carers Information and Support Service, East Riding Carer Support, Innovations in Dementia, Integrated Care Centre and has contact with many other agencies.

Assessments are offered at a variety of locations across Hull & East Riding.

Home visits are available to those who meet the Housebound Criteria.

The assessment includes a memory assessment and the gathering of other information from the person and their supporters (where consent is given).  This information may be about you, your lifestyle and life experiences and your health and any treatments you are receiving.

Following diagnosis, a range of treatment interventions can be offered to the person living with Dementia and their supporters, which for some may include medication if appropriate.
