TranslateEast Riding Care Home Liaison Team
We aim to improve the quality of life for people living with dementia in care homes across the East Riding of Yorkshire and support those who care for them.
The Team
We are a team of specialists in dementia care, complex needs and behaviours that challenge. Established in September 2019, our team includes Clinical Psychology, Specialist Mental Health Nurses with access to Community Mental Health Teams (CMHTs) and the wider multi-disciplinary team.
How to access the service
Our referral criteria: complex needs related to dementia, unstable placements and/or chronic behaviours that challenge
- CMHT’s can refer patients with the above criteria for joint working
- Crisis Team can refer patients who are no longer in need of daily crisis support and who fit the above criteria
- Acute hospital social work teams and Hospital Mental Health Liaison can refer patients who have complex needs, dementia and are fit for discharge and who need support to re-enter residential care
- Direct referrals accepted from care homes within the East Riding for patients who fit the above referral criteria
- Referrals are also accepted from other health and social care professionals for patients who fit our criteria
Opening times
Our service operates normal working hours Monday to Friday, closed bank holidays. Please note we are not a crisis service.
