Intensive Support Team

The Intensive Support Team (IST) works with service users who require more intensive support at a given time to manage risk, prevent placement breakdown or an inpatient admission.

The IST is multidisciplinary, and includes Specialist Nursing, Speech and Language Therapy, Occupational Therapy, Psychology and Healthcare Assistants. There are close links between the IST and social services both in Hull and East Riding.

The Team

The aim of the Intensive Support Team (IST) is to reduce and manage behaviours that challenge, working with the person in their own residence, to reduce the need for the use of restrictive practices, inpatient services and out-of-area/residential placements.

This is achieved through 4 core functions of support:

  • Assessment, treatment and support for individuals who display behaviour that challenges. The focus is on preventative interventions to reduce the likelihood of a crisis happening. This includes work such as Positive Behavioral Support plans (PBS), therapeutic groups and individual work.
  • Provision of support, and person specific training for other agencies supporting those individuals. PBS is integral to the work that the IST do, and the team support the creation of plans as well as offering advice and training to service providers around individual clients.
  • Coordination of transitions from inpatient and other settings by working collaboratively with other services.
  • Crisis response.
