Learning Disability Services

The service works with adults (18+ years) with a learning disability and health needs who require specialist support in the community.

There are various community services, an assessment and treatment unit (Townend Court) and a specialist nursing home for individual with profound and multiple learning disabilities that require 24 hour nursing care (Granville Court). How to access these services

The learning disability service operates an open referral system. The Community Team for Learning Disabilities (CTLD) is the first point of contact to both inpatient and community services. Individuals accessing the service have a learning disability diagnosis, are over 18 years of age or in transition from children’s services and require a period of joint working to be undertaken prior to transfer. Referrals are only accepted to the team if the individual has a physical or mental health need that requires support from a specialist learning disability service over and above what can be offered through mainstream services.

Opening times

The Community Teams for Learning Disabilities (CTLD) and Forensic Outreach Liaison Service (FOLS) operate a core multi-disciplinary service from Monday to Friday from 9am to 5pm.

The Intensive Support Team (IST) run a 7 day a week service from 8am-8pm.
