Humber & North Yorkshire Provider Collaborative Single Point of Access

The Humber & North Yorkshire Provider Collaborative Single Point of Access (H&NY SPA) accepts referrals for adult forensic mental health services in North Yorkshire, Hull & East Riding, North Lincolnshire and Northeast Lincolnshire.

The H&NY SPA aims to foster a system-wide approach by unifying referrals for secure beds and the Forensic Community Teams (FCT) across the H&NY ICS, thereby improving quality, governance, consistency, effective communication and service user experience.

The Aims of the H&NY SPA are:

  • To avoid admission wherever possible through flexible, innovative, bespoke community solutions and joint working
  • To triage, signpost and track referrals
  • To support the delivery of good quality Access Assessments & Consultations
  • To oversee bed management across all providers based on up to date clinical and operational information
  • To uphold good communication with service users, referrers and stakeholders
  • To support the outlining of clear treatment goals, explicit timescales and transfer/discharge proposals before admission
  • To understand local population needs via a datainformed approach
  • To highlight variation in practice and service user experience with a focus on quality – especially to reduce repetitive, multiple or unnecessary assessments
  • The SPA supports and oversees access assessments making decisions and recommendations considering the whole care pathway for service users, including alternatives to admission to secure care.

 

 

The area covered by the H&NY SPA includes North Yorkshire, North Lincolnshire, Northeast Lincolnshire and the East Riding of Yorkshire.

 

 

Advice for Inpatient Referrers 


Please feel free to contact the Single Point of Access (SPA) to discuss a  referral prior to completing the referral form by emailing hnf-tr.hnyadultforensicreferral@nhs.net or telephoning 01482 478702 as our SPA Co-ordinator will be happy to advise whether the person is likely to meet the Forensic Services Inclusion criteria. It is essential that the referral form is fully completed and that any additional reports or supplementary information accompany the referral. Failure to provide sufficient information will result in a delay processing the referral.

The H&NY SPA accepts referrals Monday – Friday (excluding Bank Holidays) between 9am – 5pm. The SPA Co-ordinator will triage the referral and ensure that it is forwarded to the most appropriate provider within the H&NY Provider Collaborative, where it will be discussed in their weekly referral meeting and, if accepted, will be allocated to the most appropriate clinician to lead on the assessment/consultation.

Forensic Inpatient Bed Referrals:

This form cannot be saved so will have to be completed in one session; therefore please ensure you have all information prior to starting.

Forensic Community Team Referrals:

Please telephone: 01482 478702, where the SPA administrator will take some initial details. 

When making a referral please have to hand the following information:

- Confirmation of consent and capacity to consent to the referral
- Name, Date of Birth and contact details  of the person being referred
- Basic demographics such as ethnicity and gender
- GP details
- Details of professionals and agencies involved
- Current diagnosis
- Current and historical risks

An MS Teams meeting will be arranged to discuss the referral in greater detail with our SPA clinician.
