Primary Care Mental Health Networks- East Riding of Yorkshire
The teams operate a recovery-based community mental health service for people living in Hull and the East Riding, covering a broad range of mental health problems for those individuals who do not require a secondary care service.
The Teams
The service is provided in partnership with the East Riding of Yorkshire Council other partners and Humber Teaching NHS Foundation Trust in conjunction with other internal mental health services and YOURhealth. The teams work closely with service users and carers, their families, primary and secondary care services, and other local statutory and non-statutory agencies. The team is multi-disciplinary consisting of a small range of health and social care professionals supported by administration staff.
The Primary Care Mental Health Networks aim to provide high quality, responsive and recovery-focused care and support to working age adults and older adults.
The team provides interventions which reflect an individual’s own unique needs, promoting diversity inclusion and respect for all people from all sections of society regardless of race, disability, gender, sexual orientation, culture or religion.
Our Interventions
All service users will be offered triage and assessment of their needs and strengths and will have a plan which sets out the type of care and support required to assist with their recovery. The type of interventions provided will depend on the assessed needs of the service user and take into account evidence-based guidance.
The plan will be agreed in conjunction with the service user to promote their recovery and work positively towards their autonomy away from mental health services.
