Community Mental Health Service - Hull East

Our recovery focused service in East Hull is for working aged adults who are experiencing secondary mental health illness.

The Team

The Hull East Community Mental Health Team is delivered by a multi-disciplinary team comprising a range of professional disciplines including community psychiatric nurses, social workers, care officers, occupational therapists, healthcare support workers, clinical psychologists and psychiatrists.

The team use a recovery focussed pathway model as part of our service delivery. We plan care to both measure and support progress for service users towards self-reliance, or other goals, in areas including managing mental health, physical health, living skills, responsibilities, relationships and social networks. The clinical team uses a collaborative approach with patients and their families or carers as an integral part of the work.

The team aim to:

provide a comprehensive care plan of a person’s needs, including risks and, where appropriate, physical healthcare needs, using evidence based tools;
prevent relapse; including the agreement of a relapse plan, if relevant, to support the individual:
increase education of diagnosis and symptoms with psycho-educational intervention;
prevent side effects of medication with monitoring;
promote recovery by providing interventions to help service users to live a satisfying and meaningful life within the limits of mental illness.

Evidence-based interventions will be provided including: family therapy; cognitive behavioural therapy; group work for activities and activities of daily living; training and employment; assistance in promoting independence in areas of accessing community and local services. We work closely with external agencies such as housing, employment, voluntary services and addiction services.

Regular reviews will be provided between the patients, their carer (where appropriate) and professional, as guided by the national care clusters and a person’s needs by the allocated care coordinator or case manager.

If a patient who is receiving care from our Team is admitted to our inpatient service, the team will maintain contact and be involved in ongoing care and discharge planning.

The service is also a learning environment which facilitates student learning of all professionals within the multi-disciplinary team.

This service is underpinned by legislation such as Mental Capacity Act, Mental Health Act, Mental Health Strategy 'Closing the gap: priorities for essential change in mental health', NICE guidelines, and England's mental health strategy 'No health without mental health'.

The service is commissioned by Hull Clinical Commissioning Group with use of Payment by Results and guided by the national care clusters.
