Shared Care Records - BeDigital Week 2024

On the final day of our BeDigital Week 2024, we’re looking at how we manage Shared Care Records for, not only our services, but also a wide range of health and social care organisations across the country.
Interweave began as one of only five Local Health & Care Record Exemplar (LHCRE) programmes commissioned in England, hosted by our Trust. NHS England subsequently mandated that each of the 42 ICS regions implemented a similar capability, which became known as Shared Care Records. Following this, Interweave transitioned from a LHCRE to what is now known as the Yorkshire & Humber Care Record (YHCR). The YHCR covers three local ICS regions; Humber & North Yorkshire, West Yorkshire, and South Yorkshire, with a total of around 5.7 million patients.
The aim of the Shared Care Record is to present all health and care data for a patient within a single digital interface. This is regardless of the where the data was created or is being held and can include GP Practices, Acute Trusts, Social Care, Mental Health, and Community organisations. Thereby providing the right information, to the right people, at the right time, enhancing people’s experiences and their overall care.
Our Trust not only manages Interweave, but we also provide data from our Electronic Patient Record (EPR) for other organisations to benefit from. Staff within the Trust can also view Shared Care Records through a link within the EPR.

The success of the YHCR technology has resulted in interest from other ICS regions, which led to a process of re-branding to Interweave. Interweave first began working in partnership with Leicestershire and Nottinghamshire as part of this change and have since introduced
the technology into other areas, including the Derbyshire ICS and serving a total population size of around 8 million patients.
