Get free health and wellbeing support in Goole today

Our YOURhealth service have partnered up with Cygnet Primary Care Network (PCN) to provide personal support and motivation with changing behaviours and help reduce the risk of long-term health conditions.

Patients registered with one of Cygnet PCN’s five GP practices, in Goole and the surrounding areas, can self-refer into the our health and wellbeing service to receive free one-to-one support.

The health and wellbeing support is designed to help you set achievable goals, overcome barriers, and develop a personal plan to make positive changes for the long term. It is therefore an opportunity to tailor the support to you and your individual needs.

Once your referral is accepted, our YOURhealth service’s coaching team will spend up to an hour with you at each meeting, and can meet with you weekly or fortnightly, in a way that works around your life.

  • Healthy eating
  • Weight management
  • Alcohol support
  • Sleep Hygiene
  • NHS Health Checks
  • Group support, coming soon

NHS Free Health Checks

If you’re aged 40 to 74 and living in the East Riding of Yorkshire, you may also be eligible for a free NHS Health Check delivered by the YOURhealth service. The check includes a cholesterol and blood pressure check, as well as height, weight and waist measurements and general lifestyle improvement advice and support.
