CCG9: Cirrhosis and fibrosis tests for alcohol dependent patients	Adult inpatients with a primary or secondary diagnosis of alcohol dependence have an order or referral for a test to diagnose cirrhosis or advanced liver fibrosis. Target 35%.
CCG10a: Routine outcome monitoring in CYP and perinatal mental health services	Achieving 40% of children and young people and women in the perinatal period accessing mental health services, having their outcomes measured at least twice.
CCG10b: Routine outcome monitoring in community mental health services	Achieving 40% of adults and older adults accessing select Community Mental Health Services, having their outcomes measure recorded at least twice.
CCG1: Use of Anxiety Specific Disorder Measures (ADSMs) in IAPT	65% of Improving Access to Psychological Therapies (IAPT) referrals with a specific anxiety disorder problem descriptor finishing a course of treatment having paired scores recorded on the specified ADSM.
CCG12: Biopsychosocial assessments by Mental Health Liaison Services	Achieving 80% of self-harm7 referrals receiving a biopsychosocial assessment concordant with NICE guidelines.

PSS6: Delivery of formulation or review within 6 weeks of admission, as part of a dynamic assessment process for admissions within Tier 4 CYPMH settings	The proportion of inpatients with a formulation or review, undertaken within 6 weeks of admission, that has been shared in the appropriate format with the young person, carers and community key workers as part of a dynamic assessment process. Target 80%.
PSS7: Supporting quality improvement in the use of restrictive practice in Tier 4 CYPMH settings	Achieving 80% of restrictive interventions (RI) records submitted to the Mental Health Services Data Set (MHSDS) with all mandatory and required fields completed.

CCG14: Assessment, diagnosis and treatment of lower leg wounds	Achieving 50% of patients with lower leg wounds receiving appropriate assessment diagnosis and treatment in line with NICE Guidelines.
CCG15: Assessment and documentation of pressure ulcer risk	Achieving 60% of community hospital inpatients aged 18+ having a pressure ulcer risk assessment that meets NICE guidance with evidence of actions against all identified risks.
